Definition การเจ็บครรภ์คลอดก่อนกำหนด (Preterm labor) : ภาวะที่มดลูกบีบตัวอย่างสม่ำเสมอ (Regular uterine contraction) ทำให้ปากมดลูกเปิด (Cervical change) ก่อนอายุครรภ์ 37 สัปดาห์ การคลอดก่อนกำหนด (Preterm birth): ทารกที่คลอดอายุครรภ์ 20 -37 สัปดาห์ ซึ่งการกำหนดอายุครรภ์ขึ้นอยู่กับแต่ละสถาบัน โดยแยกย่อยออกเป็น Late : >34 wks moderate : 32-34 wks very : 28-32 wks extremely : <28 wks Spontaneous preterm birth : การคลอดก่อนกำหนด ที่ไม่มีข้อบ่งชี้ (maternal and fetal condition) Gynecologists TACoOa. Definition Preterm labor : การเจ็บครรภ์ก่อนอายุครรภ์ครบ 37 สัปดาห์ โดยมีเกณฑ์การวินิจฉัยดังต่อไปนี้ 1. มีการหดรัดตัวของมดลูกอย่างน้อย 4 ครั้งใน 20 นาที หรือ 8 ครั้งใน 60 นาที ร่วมกับมีการเปลี่ยนแปลงของปากมดลูก 2. ปากมดลูกเปิดมากกว่า 1 cm 3. มีความบางตัวของปากมดลูกร้อยละ 80 หรือมากกว่า

Diagnosis of Preterm labor Symptoms : regular contraction & cervical change Cervical change : cervical dilatation >= 3 cm Vaginal Cervical length Short cervical length is associated with an increased risk of PTB Fetal fibronectin glycoprotein secreted by the chorionic tissue. acts as a biological glue which binds blastocyst to endometrium. normally present in cervicovaginal secretions upto 20-22 wks Elevated levels of fFN (>50 ng/mL) after 22 weeks gestation-increased risk of PTB

Antenatal corticosteroid การให้สเตียรอยด์ในสตรีที่มีอาการเจ็บครรภ์คลอดก่อนกำหนด จะช่วยลดอัตราการตายของทารกแรก คลอดและภาวะ respiratory distress syndrome (RDS)ลดลง และช่วยลดภาวะ intraventricular hemorrhage The National Institutes of Health (NIH), American College of Obstetricians and Gynecologists (ACOG) และRoyal College of Medicine ต่างก็แนะนำให้ใช้ steroid ในสตรีที่มีอาการเจ็บครรภ์คลอด ก่อน 34 สัปดาห์ (26-32 wks) ชนิดของสเตียรอยด์ มี 2 ชนิด ได้แก่ Betamethasone (12mg; 2times q 24) Dexamethasone (6mg; 4times q 12)

Antibiotics Routine administration of antibiotics in premature labor without premature rupture of the membranes is not recommended, because the rate of maternal infection is lower ,nor reduction of the neonatal complications. (Subramaniam et al.,2012) Use for PPROM (24-32 wks) and GBS carrier status (Prevention of perinatal group B streptococcal disease —revised guidelines from CDC, 2010)

Recommended management of preterm labor 1.ยืนยันภาวะเจ็บครรภ์คลอดก่อนกำหนดตามเกณฑ์การวินิจฉัยข้างต้น 2.ในกรณีอายุครรภ์<34 wks ที่ไม่มีภาวะแทรกซ้อนของมารดาและทารกที่ต้องให้คลอด close observation monitor uterine contraction และ FHR ตรวจภายในเพื่อประเมินการเปลี่ยนแปลงของปากมดลูกตามความจำเป็น ให้ corticosteroids เพื่อกระตุ้น lung maturation พิจารณาให้ magnesium sulfate นาน 12 – 24 ชั่วโมงเพื่อหวังผล fetal neuroprotection ให้ยายับยั้งภาวะมดลูกหดรัดตัวหากยังไม่เข้าสู่ระยะ advanced labor

3. ในกรณีอายุครรภ์ > 34 wks close observation monitor uterine contraction และ FHR ตรวจภายในเพื่อประเมินการเปลี่ยนแปลงของปากมดลูกตามความจำเป็น 4. ในกรณี active labor ให้ยาปฏิชีวนะเพื่อป้องกัน neonatal group B streptococcal infection

Take home message There is no indication in routine clinical practice for continuing tocolytic therapy for more than 48 hours. Except in some cases (e.g., placenta previa hemorrhage, amniotic sac prolapse). (Schleußner.2013) Using multiple tocolytic drugs associated with a higher risk of adverse effects and should be avoided. (De Heus et al.,2009) There is no evidence that bed rest actually lowers the rate of preterm birth. (Crowther and Han. 2012)
