Decontamination What is decontamination Decontamination (sometimes abbreviated as decon, dcon, or decontam) is the process of removing contaminants on an object or area, including chemicals, micro-organisms or radioactive substances. This may be achieved by chemical reaction, disinfection or physical removal. It refers to specific action taken to reduce the hazard posed by such contaminants, as opposed to general cleaning What is aseptic technique Aseptic technique is the process of utilizing various techniques to reduce the risk of microorganism transmission When is it necessary to decontaminate items After the items have been used or have entered the sterile field If instruments have fallen on the floor. If your sterilizer load has failed items must be returned to decontam If the item you receive on the prep side appears dirty If items such as loaners trays have been received from the outside If a tray has been opened for any reason.

Medical washer Disinfector how does it work When are they used? Washer disinfectors are used for automated cleaning of surgical instruments or medical devices. They offer increased productivity compared to staff manually cleaning and provide a consistent, repeatable cleaning process so staff can be sure devices are thoroughly cleaned every time. How do they work? A typical washer disinfector cycle consists of multiple phases including pre-wash, several washes with specific cleaning agents under controlled conditions, one or several rinses, and a thermal rinse appropriate for the types of items contained in the load.

VERIFY® All Clean™ Test Washer Indicator Title: Monitoring and verifying automated instrument washer/disinfector cycles with the use of the Verify All Clean Test Washer Indicator. Purpose: Health care personnel should perform verification testing on all mechanical cleaning equipment as part of the overall quality assurance program. Methods of verification include: directly testing individual instruments for residual soils, employing a test device that is a consistent and repeatable challenge to the cleaning effectiveness of the equipment; and monitoring critical parameters to evaluate the performance of the mechanical cleaning equipment. Mechanical cleaning equipment should be tested upon installation, each day that it is used, and after major repairs. (ANSI/AAMI – ST79:2017, 13.2) Procedures: NOTE: The washer/disinfector loads will be monitored with the Verify All Clean Test Washer Indicator. Equipment Inspection: 1) Follow manufacturer’s guidelines concerning inspection of the automated washer/disinfector. This can include, but is not limited to, spray arm rotation, spray arm clogging, screen maintenance, and chemistry supply lines leading from the source to the washing equipment. 2) Ensure cycle parameters are set to desired levels. Initiating the Cycle: 1) Ensure the All Clean Test Indicator strip is within expiration date. 2) Ensure the All Clean Test Holder is clean and dry. 3) Place one All Clean Test indicator into the holder ensuring it is centrally placed and not protruding from either side. 4) At the beginning of each day, the device should be run in a complete EMPTY load to establish a control. 5) Place the device (indicator inside holder) into a wire basket. 6) Device can be placed on every level of the rack being processed. 7) Orientation of the device should be with one of the soil stains flat on the wire basket and the other soil stain orientated vertically. See Verify All Clean Test Indicator Wall Chart for Illustration. 8) Load the rack into the automated washer chamber. 9) Close the chamber door, and start the cycle. 10) Select the standard Instrument Cycle from the washer control. Upon Completion of the Cycle: 1) Review the cycle print out to ensure all the processing parameters were met (i.e. time, temperature). 2) Once the chamber door opens, ensure the rack exits the chamber. 3) Carefully remove the device from the basket and carefully remove the All Clean Test Indicator from the holder. 4) Caution should be used, as the device may be hot. 5) Inspect the indicator for evidence of soil by placing the plastic film against a white background (e.g. the background of the All Clean Test Indicator Wall Chart). 6) Compare the results against the sample outcomes provided on the All Clean Wall Test Indicator Chart. 7) If evidence of soil remains on the indicator, the result is a FAIL. 8) If no evidence of soil remains on the indicator, the result is a PASS. 9) Consult the All Clean Test Indicator Wall Chart for troubleshooting and diagnosis of the failure. 10) Record results on a log sheet. 11) File the cycle print out and All Clean Test Indicator Strip in accordance with departmental procedures.

Steps for manual cleaning of trays prior to Automated washing Step 1 take instruments out of tray and string them on a stringer this helps to keep the instruments in an open position so the washer can effectively clean in the hard to reach areas. While removing instruments from tray check for any bio burden that may need to be brushed off the instruments. Do not dump instruments out of tray as it could do damage to the instruments. Step 2 Take instrument tray to sink to soak and clean any dirty instruments in need of washing. Check all tissue forceps and anything in the tray that could harvest bio burden. Example bone in rongeurs. Also brush and flush any lumen items such as Suctions Use Steris channel cleaning brush as shown. Flush using Pure Processing Flush pump Step 3 Trays are to soak in enzymatic solution for a minimum of 2 minutes prior to going through the ultrasonic Step 4 place trays in ultrasonic machine. Hook up any lumen items to the ultrasonic machine to be flushed. Step 5 Place tray in washer disinfector hook up any lumen items to be flushed Dirty Rongeur Dirty Suction

This will allow the cycle to start automatically Step 2 In the event you have to select cycle manually if shown on the Screen select appropriate cycle otherwise scroll to cycle example Eye cycle. Press eyes and cycle will start. Vision manifold rack has the ability to flush lumen items using the irrigation ports and flexible hoses. See picture to left.

How does ultrasonic cleaning work When are they used? Ultrasonic cleaners provide a form of mechanical cleaning typically used for medical devices or surgical instruments that may be too delicate for a washer disinfector and/or have hard-to-reach areas. How do they work? Ultrasonic cleaners use acoustic cavitation to clean surgical instruments and devices. Cavitation forms microscopic air bubbles that can reach small crevices and hard-to-reach areas on a device such as fine serrations or box lock joints, imploding on the instrument’s surface.

How does a cart washer work When are they used? Hospital cart washers provide efficient cleaning, disinfecting, and drying of surgical instrument case carts, containers, utensils, and other reusable non-critical items used in hospitals and healthcare facilities. How do they work? Cart washers work by using various cleaning phases – wash, rinse, thermal rinse, dry, etc. to provide low- to intermediate-level thermal disinfection of case carts and other items. Teeclean Automated probe disinfector What is Teeclean and how does it work? TEEClean provides both a scientifically verified and repeatable method for cleaning soiled TEE probes. Incorporated in TEEClean is TEEZyme®, an enzymatic solution that is injected into the cleaning and disinfection reservoir once a soiled TEE probe has been properly inserted. The TEEClean microprocessor controller then monitors all parameters of the cleaning step to ensure completion prior to high-level disinfection. After cleaning, TEEClean begins the validated and repeatable high-level disinfection process.

Medical decontamination sink When are they used? Decontamination sinks are used for manual cleaning of medical devices and surgical instruments. How do they work? Typically available in a two- or three-bay configuration, staff using decontamination sinks must follow a multi-step process that involves immersing the device in a cleaning solution and using a physical action to wipe, brush, and/or flush the device.

Removing Scopes from Medivator Step 1 Validate a pass or fail on the Rapicide test strips by gathering solution from the side of the medivator being used. Press pass or fail on medivator Step 2 place a new set of gloves on and a blue cover gown. You must use hand sanitizer prior to dawning new gloves Step 3 Place clean scope in gray cleanascope container with lid on so to not recontaminate scope Step 4 bring scope to pass through window open lid so the person on prep side can grab clean scope and place in new cleanascope container that has been lined with clear lining.

Air Filter Replacement Acquire new inline filter assemblies (See Appendix B for replacement part numbers). 2. Remove the right and left blue-colored side shrouds and locate the filters. 3. Move the filter away from the machine to allow access to the fittings . Figure 1 Filter Location Figure 2 Tubing Removal 4. Press on the push-in fitting to release the outlet tubing.
